General Motors Life Insurance Inquiry

I have a letter from the National Benefits Center in Louisville dated 2004, indicating that my dad had some life insurance. Unfortunately, the phone number on the cover letter is no longer in service. We’ve received a $10,000 payment from Met Life, but I’m aware that there’s a $34,000 basic group life policy for salaried employees, which appears to be a separate issue. I’ve already contacted Met Life and Fidelity, but they haven’t been able to find any information. Does anyone know of any other resources or contacts I could reach out to in order to help my mom? Any assistance would be greatly appreciated. Thank you!
