Hello everyone! I have a question regarding documentation practices. I currently work as a nurse in a long-term care (LTC) facility, having transitioned from home health. In home health, we had to provide thorough documentation to receive payment, particularly since we mostly dealt with Medicare. However, in my current role, it seems that nurses primarily record vital signs and complete minimal documentation. The facility owner has mentioned that it’s sufficient to only show that a patient was seen. Is this accurate? Does the quality of our documentation really not matter as long as we can prove patient contact? I’d love to hear your thoughts on this!
