Searching for Life Insurance Information from General Motors

I have a letter from 2004 from the National Benefits Center in Louisville indicating that my dad had some life insurance. Unfortunately, the phone number on the cover letter is no longer in service. We did receive a $10k payment from MetLife, but it appears this is related to the $34k basic group life insurance for salaried employees, which seems to be something different. I’ve already contacted MetLife and Fidelity, but they haven’t been able to locate any additional information. Does anyone know anyone else I can reach out to for help in finding out if there’s anything available for my mom? Thank you!
